Register as a New Patient
We welcome new patients from the town and surrounding area, please take a look at our practice boundary map.
Registering with us is simple, you can download and complete the forms from our website or register directly online via the following link - https://gp-registration.nhs.uk/P81132
If you have never been registered with a GP in England, as part of our registration process, we ask that you supply 2 forms of identification to show your eligibility to be registered with an NHS practice. This can include a passport, driving licence, utility bill (dated in the last 3 months but not including a mobile phone bill), birth certificate (for children) or a document to show your right to reside in the UK.
We’ll contact your old GP and take care of everything else for you.

Accepted ID Documents (where required) Personal Identification: Passport or Identity Card, birth certificate, driving licence, marriage certificate, NHS Medical Card, student ID card. Proof of Address: Bank statement, rent book / tenancy agreement, council tax statement, wage slip (if it includes home address), council tax statement, utility bill (dated within last 3 months), official letter (Home Office / DWP), driving licence. We need two forms of ID. So your driving licence, for example, can't be used for both your Personal Identification and your Proof of Address. If you do not have any of the above documents, we should still be able to register you as long as you live within our practice boundary. Please speak to the receptionist about your situation. |
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Named Accountable GP
As part of the commitment to more personalised healthcare for patients, NHS Employers and the General Practitioners Committee of the British Medical Association have agreed that all patients will have a named accoutable GP
- Waterfoot Medical Practice will ensure that there is a named accountable GP assigned to each patient
- New patients will be allocated a GP at the time of registration
Your named accountable GP will be the same as your registered GP however this does not affect your ability to see any GP of your choice as you currently do.
You do not need to take any further action, but if you have any questions, or wish to discuss this further with us, please contact our reception team.
